Before you start

This form is required to be completed by any exercise user groups that want to run fitness classes or training sessions in a Hobsons Bay reserve or park. Before proceeding with this application, each group/owner is required to attach the following documents. You will need to have this available to upload before proceeding

  • Certificate of currency ($10 million public liability insurance)
  • Current first aid training certificate (expires after 3 years)
  • Current CPR training certificate (expires after 1 year)
  • Qualifications (eg: Cert III or IV)
  • Certificate of registration with a peak sporting body OR agree to Fitness Australia's code of conduct
  • Review and agree to the Terms and Conditions of a Personal Training Permit in Hobsons Bay
